We are now offering virtual visits with your provider from the safety and comfort of your home!
Community of Hope now offers convenient telehealth visits with your provider either over the phone or by video. This visit is like a regular in office visit where you will have the opportunity to discuss your medical concerns with your provider.
COH will handle certain visits by telehealth. Some examples include: follow up for your chronic disease visit, check up on medications and prompt care visits.
Prompt care visits will be scheduled on the same or next day that you call and will address problems such as cold and flu symptoms, allergies, sore throat, rash or hives, nausea, headaches, known urinary tract infection, known yeast infection, constipation.
We will not do a video visit if you are having severe symptoms.

Preparing For Your Visit
Please be prepared for your visit by being in a quiet and private place prepared to talk with your provider at the start of your appointment time. If you are unable to be in a safe quiet place during your visit your appointment may not be able to take place. The visit may last 30 minutes.
A link for the video appointment will be sent to you via text or email, depending on the preferences you have indicated. Please make sure:
- We have a good email on file for you
- We have your correct cell phone number
We can include video if you have a smartphone, tablet, or computer with a camera and microphone. If you do not have any of these or don’t prefer a video appointment, we will call you by phone.
If we schedule you for a video telehealth visit, a text will be sent shortly before the visit with a link to Doxy.me to start the visit. You should join the visit by clicking the link. Please let us know if you need to cancel ahead of time just like a regular appointment. This visit will be billed just like a regular appointment.

How do I join a TeleVisit?
On your smartphone or computer: Click on the text or email link you receive from your doctor’s office. No login required.
On the day of the Appointment:
- Set aside a secure and private location for your appointment.
- “Arrive early” by clicking the link within the email reminder or text message your doctor’s office sent you.
- Fill out any patient questionnaires and vitals if applicable (answers are not mandatory on either form, but fill in what you can).
- Click “Proceed” to check your computer’s speed and ability to support a TeleVisit.
- Click “Start TeleVisit” to enter the virtual waiting room.
- Your physician will begin the face-to-face TeleVisit and disconnect when the visit ends.

No. You must first come in for your new patient appointment to meet the provider and then we can schedule a physical.
You may make refill requests using the patient portal at any time. You can also reach our prescription refill line by calling the main number and following the prompts. Please leave a message with the following information: Your name, birth date, phone number, medication needed, pharmacy name and phone number. For diabetic supplies, include the type of glucometer you currently use.
Let the receptionist know that you wish to sign up for the Patient Portal and we will send instruction to your email account. Once you have set up a username and login, you can also use an easy app to access the Portal from your smart phone or the internet. Search for the Healow app. You can also access the Patient Portal from our website.
The portal will help you recover a lost password or login. You can also contact a receptionist for help.

We often have live interpreters for Spanish and Amharic. In addition, we are able to accommodate any language via the Language Line services.
Most insurance plans require a referral for specialty care. Referrals must be requested and approved by your Community of Hope provider. If you come in for a visit, we will have the referral ready for you before you leave. If you send a message via the patient portal to your provider, let your provider know if you prefer to pick it up or have it mailed. Make sure to give your provider your correct telephone number and address. Once your referral is complete, you will be notified. Referrals may take approximately one week to process and complete.
To request medical records, please ask a receptionist for a Release of Information form or request one through Patient Portal. Once you complete and sign the form, our medical records specialist will process your request within 30 days. Community of Hope wants to ensure that every young adult has access to accurate information about their sexual health. Same day and confidential appointments are available for adolescents and young adults ages 12-24. Services include: general medical care, pregnancy testing, counseling, STD and HIV testing, HPV vaccine, and birth control. Our counselors will help you make a decision that is right for you. IUD’s and implants are available for teens aged 12-19 years old free of charge.

Each of our health centers is a community health center, also called a Federally Qualified Health Center (FQHC). That means that we are a part of a national network of community health centers funded in part by the federal government to make sure that high quality health care is available to everyone. We also receive our malpractice insurance from the federal government through the Federal Torts Claims Act. Community of Hope is fully deemed in this program.
