We are proud to serve families and individuals along the homelessness continuum using the Housing First model. Solving homelessness is more than ensuring a roof over a head or a safe bed to sleep in. It’s meeting families where they are and working with their fundamental strengths to achieve their goals.

We help families at risk of losing their housing find stability. We provide targeted case management, financial assistance, and connections to mainstream resources so that they do not need to enter the shelter system.

Our case managers serve as supporters and advocates for clients – helping them identify their goals and design detailed plans with realistic timelines. Wide-ranging supportive services include:

- Budgeting and financial management
- Rental and utility assistance
- Referral for legal services
The Homelessness Prevention Program, through expert case management and mediation services, helps families stay stably housed and allows them to remain connected to their family and community support systems.
If you know a family encountering a housing crisis that they cannot resolve on their own, they can access homelessness prevention advice and program assistance referrals exclusively through the Virginia Williams Family Resource Center, the central intake for DC’s coordinated homelessness assistance system for families.
Welcome! Community of Hope offers temporary solutions for both families with children and individuals in a few site-based programs across Washington, DC. With a safe place to stay, families and individuals are able to work to achieve their goals on a new home, employment, health, and a strong family connection.
We offer short-term housing with 24-hour support for up to 50 families. Children of all ages can play and do homework while we help parents quickly develop a plan to move into their own home.
Families are referred through Virginia Williams Family Resource Center, the central intake for DC’s coordinated homelessness assistance system for families. Community of Hope is pleased to bring our experience in ending family homelessness to The Triumph – short term family housing. The Triumph is where families are provided:
- Access to services in their own community
- 24-hour support as families plan to move to more permanent housing.
- Connection to Community of Hope’s largest and most robust healthcare center, providing essential services such as: medical, dental and emotional wellness, homelessness prevention, maternal and child health, and home visiting for families with young children.
Our Bridge at Girard Street location is piloting a bridge housing program for approximately 40 individuals who have been unsheltered or living in tents throughout Washington, D.C. We offer a wide range of supportive services and resources including:
- Goal-oriented case management
- Assistance on finding an affordable apartment
- Connection to employment, training and behavioral health services

Housing

Re-housing
We work with families who’ve recently moved out of shelter and into their own homes in our Rapid Re-Housing program. Families receive short- to medium-term rental assistance and supportive services from our case managers. In addition, we connect families with employment services and resources in their communities.
Putting families on a path of self-sufficiency
We address housing, employment, and educational needs. Our Rapid Re-Housing program serves up to 400 families at any given moment while our team also has a leadership role in developing this best practice model in Washington, DC.

With our support, families and individuals experiencing chronic homelessness find and maintain permanent housing through intensive case management, rental assistance, and specialized care for children and youth. With a holistic approach, we focus on all areas of people’s lives.
A Holistic Approach to Family Care
These programs are based on Housing First, a nationally successful model that focuses on helping people move into and sustain permanent housing – along with a variety of services to promote stability, self-sufficiency and well-being. We place families and individuals into permanent homes and provide them with the tools they need to focus on their life goals, including:
- In-home, goal-oriented case management
- Assistance on finding an affordable home
- Connection to employment and training
- Navigation through school and life challenges for their children
- Pairing eligible youth with volunteer mentors
- Connection to community resources
Site-Based Housing Partnerships: In a public, private, and nonprofit partnership – we provide apartment-style permanent supportive housing to families and individuals.

Housing Questions
Community of Hope has several housing programs, serving both families and individuals. The programs that serve families include Homelessness Prevention, Short Term Family Housing (The Triumph), Rapid Rehousing, and Permanent Supportive Housing. The programs that serve individuals include Bridge Housing (The Bridge at Girard and Hope Apartments) and Permanent Supportive Housing.
Community of Hope’s Homelessness Prevention Program works to stabilize families at risk of becoming homeless through specialized services and connection to other community resources. Families accessing Homelessness Prevention services have a place to stay for one to 30 nights and work with the case managers to find stable housing.

Rapid Rehousing, also known as the Family Stabilization and Rehousing Program (FRSP), is a time limited housing subsidy with case management program that helps families who have experienced homelessness get access to resources and work on goals in order to increase housing stability. Families rent apartments throughout the city and work with case managers on their housing stability, employment, and/or education goals, and also work to get connected to community resources that will help them maintain stability after the subsidy ends. Community of Hope’s Rapid Rehousing program serves up to 400 families at any time.

Permanent Supportive Housing is a long-term housing subsidy and case management program for families and individuals who have a chronic disabling condition, limited earning potential, and a need for case management services. The Permanent Supportive Housing program offers a long-term subsidy and intensive in-home supportive services to help each family and individual find and maintain a stable home. Community of Hope offers both Tenant-based and Project-based programs and serves up to 162 families and ??110 individuals.

Bridge Housing is a short-term shelter program for individuals who have been matched to permanent housing but have yet not found an apartment. Community of Hope has two Bridge Housing programs, Girard Street and Hope Apartments, that serve up to 6065 individuals. These programs allow for outreach and permanent housing case managers to work alongside the shelter case managers and the individual to quickly help them complete the subsidy application and find stable housing.
Individuals access Bridge Housing by referral from the Department of Human Services.

Insurance Questions
For emergencies or if you feel that you are in danger or if you feel like hurting someone else, please dial 911 or contact the Department of Mental Health 24-Hour Access Helpline at 1.888.793.4357.
During the daytime, you can call or visit us to see a counselor who can help talk you through the crisis and evaluate your options, which may include counseling or medication. Community of Hope offers counseling services, as well as short term crisis support.
We provide prenatal and pediatric care at all three of our healthcare centers!
Patients receiving prenatal care have a variety of options to consider, which are partially influenced by their type of insurance. Your provider can talk through your preferences and arrange care that works for you and your family. When under the care of one of our midwives, you will have the option to deliver at our birth center or at Washington Hospital Center. Learn more about our birthing services.
Hours and Location Questions
Community of Hope offers “open access” scheduling for medical visits and regular dental cleanings and exams. This means that we will schedule your visit the same day that you call or within three days. Our patients find this very convenient. We will also call to remind you when it is time to schedule a follow up appointment, so you can plan a visit that is convenient for your schedule.
If you are an established medical patient and need to speak with a provider after hours about an urgent issue, call your regular site number and an operator will take a message to be sent to our on-call provider. The on-call provider will give you medical advice and next steps. If your condition is life-threatening, please call 911 or head to the nearest emergency room. This service is not available for individuals who have not yet been seen at our offices. Please note that the on-call provider cannot schedule appointments, refill medications or review your lab results. If you sign up for the online patient portal, you can handle these items at your convenience.
Established patients can get their TB tests. TB tests are given until 4:00 PM every day except Thursdays and before long weekends. Once you receive your TB test, you must come back to have it read 2-3 days later.
