Community of Hope is committed to serving young families learn best practices of giving birth, taking care of babies, and what to expect in the upcoming weeks on your successful journey on being a parent. Our caring team provides a whole range of supportive services for the whole family.
Get Involved
Starting in the 6th month of pregnancy (around 26 weeks), pregnant moms join the CenteringPregnancy® group.
Participants benefit from:
Interactive learning and fun discussion in a relaxed setting with other moms due at the same time as you
Education about labor, breast/chest-feeding and basic newborn care
Games, food and chances to win prizes or receive gifts for your baby
Continue to have one-on-one checkups with the midwife during group
Centering is currently offered at the Family Health and Birth Center and Conway Health and Resource Center.
Parents of children under three can benefit from our early childhood home visiting program. These free services provide pregnant women, new moms and their families with the support and encouragement they need. We help you strengthen the bond between you and your child right in your home. We come to you!
With early childhood home visiting, you will receive:
- Educational and fun home visit sessions (for those who qualify)
- Support for when you feel tired or worried
- Tips on how to care for and bond with your baby
- Help with getting daycare, WIC, medical care and other resources
- Information about birth control and breastfeeding
- Food and fun at parent support group sessions for mom and dad
- Diapers and baby supplies when needed
Our counselors are here to help answer any questions you may have about your reproductive health. You can receive services in a supportive and confidential setting:
- Pregnancy planning and testing
- Birth control (free IUDs and implants for interested teens and some available same day)
- Emergency contraception
- Sexual health education
- Confidential teen visits
- Confidential HIV/AIDS and STI testing and counseling
- HPV vaccine

No. You must first come in for your new patient appointment to meet the provider and then we can schedule a physical.
You may make refill requests using the patient portal at any time. You can also reach our prescription refill line by calling the main number and following the prompts. Please leave a message with the following information: Your name, birth date, phone number, medication needed, pharmacy name and phone number. For diabetic supplies, include the type of glucometer you currently use.
Let the receptionist know that you wish to sign up for the Patient Portal and we will send instruction to your email account. Once you have set up a username and login, you can also use an easy app to access the Portal from your smart phone or the internet. Search for the Healow app. You can also access the Patient Portal from our website.
The portal will help you recover a lost password or login. You can also contact a receptionist for help.
We are committed to providing quality information and services to Limited-English Proficient individuals. We understand that accurate communication is essential to providing meaningful and competent care. All language services will be provided free of charge.

We often have live interpreters for Spanish and Amharic. In addition, we are able to accommodate any language via the Language Line services.
Most insurance plans require a referral for specialty care. Referrals must be requested and approved by your Community of Hope provider. If you come in for a visit, we will have the referral ready for you before you leave. If you send a message via the patient portal to your provider, let your provider know if you prefer to pick it up or have it mailed. Make sure to give your provider your correct telephone number and address. Once your referral is complete, you will be notified. Referrals may take approximately one week to process and complete.
To request medical records, please ask a receptionist for a Release of Information form or request one through Patient Portal. Once you complete and sign the form, our medical records specialist will process your request within 30 days. Community of Hope wants to ensure that every young adult has access to accurate information about their sexual health. Same day and confidential appointments are available for adolescents and young adults ages 12-24. Services include: general medical care, pregnancy testing, counseling, STD and HIV testing, HPV vaccine, and birth control. Our counselors will help you make a decision that is right for you. IUD’s and implants are available for teens aged 12-19 years old free of charge.

Each of our health centers is a community health center, also called a Federally Qualified Health Center (FQHC). That means that we are a part of a national network of community health centers funded in part by the federal government to make sure that high quality health care is available to everyone. We also receive our malpractice insurance from the federal government through the Federal Torts Claims Act. Community of Hope is fully deemed in this program.
