You may make refill requests using the patient portal at any time. You can also reach our prescription refill line by calling the main number and following the prompts. Let the receptionist know that you wish to sign up for the Patient Portal and we will send instruction to your email account. Once you have set up a username and login, you can also use an easy app to access the Portal from your smart phone or the internet. Search for the Healow app. You can also access the Patient Portal from our website.

The portal will help you recover a lost password or login. You can also contact a receptionist for help.

Most insurance plans require a referral for specialty care. Referrals must be requested and approved by your Community of Hope provider. If you come in for a visit, we will have the referral ready for you before you leave. If you send a message via the patient portal to your provider, let your provider know if you prefer to pick it up or have it mailed. Make sure to give your provider your correct telephone number and address. Once your referral is complete, you will be notified. Referrals may take approximately one week to process and complete.

To request medical records, please ask a receptionist for a Release of Information form or request one through Patient Portal. Once you complete and sign the form, our medical records specialist will process your request within 30 days. If you are calling from another provider’s office, please fax the request to 202.232.8494.
