Nicole is 23 years old and a trauma survivor. She and her family have been patients at Community of Hope for over a decade. This long, deep relationship created a foundation of trust, helping us support this fragile young woman through multiple crises.
Three years ago, her first real relationship left her pregnant and HIV positive. Her prenatal care provider at Community of Hope referred her for high risk care, preventing her son from being infected with HIV.
However, when he was a few months old, her son was diagnosed with a developmental disorder. Even at the expense of her own health, Nicole is a a dedicated mother to her son. She works hard to keep her HIV under control but her mental health has suffered, which at one point resulted in a crisis requiring hospitalization.
Nicole’s medical providers, including the dedicated Laura Worby, connected her with our behavioral health team for help. We identified a program designed to provide home health care services specifically for children with special needs, including assistance with activities of daily living such as bathing, feeding, and medication. Receiving the services she needs to help care for her son made a huge impact.
She now maintains her much needed medical and behavioral health appointments. She is now devoting more time to her own health and development as a young adult, such as enrolling in school or job training. Having the medical and behavioral services teamed together in one location means we are able to provide a personalized approach for Nicole.
