Executive Summary
Community of Hope, Inc. (COH) is committed to protecting your privacy, and that of all our donors, subscribers, and the people we serve. Our goal is to maintain the security of interested parties. This Web Privacy Policy serves to inform you of what Personal Data (see definition below) we may collect about the users of our website, www.communityofhopedc.org (the “Site”), and how we may use that information. The key statement is that we will not trade, sell, or rent Personal Data gathered from our Site. Personal Data may be used to send users information about Community of Hope. See Section II for further details.

What Information Will be Collected
A. Personal Data
Personal data collected from Users include the User’s name, postal address, home, office, and mobile telephone numbers, email address, the content of your email, credit or debit card information, job title, and employer information (“Personal Data”). You can visit the Site without disclosing any personal data about yourself. However, to sign up for the newsletter, send an email to COH, donate via the Site, and, in some cases, participate in other activities (“Services”), you may voluntarily disclose personal data. We may use this information for specific, limited purposes. You may always “opt out” at any time if you do not wish to receive our messages.
To collect, store, organize and use your Personal Data, COH may utilize Google Analytics or fundraising software, such as Blackbaud’s Raisers Edge or Online Express. This software adheres to the Payment Application Data Security Standard (PA-DSS), which allows it to be fully Payment Card Industry Compliant. The software does not store credit card information. In addition, patient information available via the Patient Portal is stored in our electronic medical records system, eClinicalWorks. No information is shared between eClinicalWorks and other databases utilized by this site. Patients may choose to sign up for newsletters, send emails, donate, and participate in other
services. During these interactions, users will not be asked to disclose whether or not they are a patient. Users may always “opt out” at any time if receiving messages is no longer desired.
The Site contains links to other Community of Hope managed sites and third-party sites. For example, links is provided to healow app and the Patient Portal, a Community of Hope managed site, which provides services for patients and contains information related to our health care provisions. Community of Hope is governed by the Health Information Portability and Accountability Act of 1996 (HIPAA) regarding patient privacy. For more information, please see our Notice of Privacy Practices.
B. Web Server Logs
Web server logs provide information recorded by the web servers about when, how, and which visitors are using a website. When you visit our Site, we may track information to administer the site, analyze site statistical metrics, and help diagnose problems with our server. Examples of information we may track include: your Internet protocol address (IP Address); the kind of browser or computer you use; the number of links you click within the site; the state or country from which you accessed the site; the date and time of your visit; the name of your Internet service provider; the web page you linked to our site from (“referrer” such as a search engine); information regarding any errors or functionality problems you experienced (“error logs”); and/or the pages you viewed on the site and how long you spent on each page (collectively “Usage Data”). Error logs are used to improve system performance. The Usage Data is statistical information that is not identified with any particular user and are all commonly gathered statistics provided by Google Analytics.
C. Use of Cookies
COH may use cookies to personalize or enhance your user experience. A “cookie” is a small text file that is stored by your browser on your computer’s hard drive. The use of cookies enables COH to obtain information about your use of the Site, such as whether you have visited the Site more than once and how you found us.
Cookies remind COH who you are and your preferences for our website based on what you’ve done and told us before. The “cookie” is placed on your computer and is read when you return to our website. Cookies let us take you to the information and features you’re particularly interested in. They also let us track your usage of www.communityofhopedc.org so we know which parts of our site are most popular. Cookies cannot be used to run programs or deliver viruses to your computer. Cookies are uniquely assigned to you, and can only be read by a web server in the domain that issued the cookie to you. One of the primary purposes of cookies is to provide a convenience feature to save you time. For example, if you personalize a web page or navigate within a site, a cookie helps the site to recall your specific information on subsequent visits. This simplifies the process of delivering relevant content and eases site navigation by providing and saving your preferences and login information as well as providing personalized functionality.
Users have the ability to accept or decline cookies. Most web browsers automatically accept cookies, but you can usually modify your browser settings to decline cookies. If you reject cookies by changing your browser setting then be aware that this may disable some of the functionality on our Site.
How Your Information Will be Used
We do not trade, sell, or rent our mailing lists or Personal Data gathered from our Site. We may use the Personal Data gathered to send users information about Community of Hope only in connection with events for which COH is a participant or beneficiary. We will not disclose Personal Data to third-parties except where we are required by law.
COH is governed by HIPAA regulations regarding the use and disclosure of Personal Health Information (PHI). Use and disclosure of PHI is detailed in the Notice of Privacy Practices. COH does not knowingly disclose patient information to third parties. In some cases, Personal Health Information does include Personal Data.
To evaluate the Site, COH may collect, store, and analyze statistical information such as Usage Data. COH may also use your Personal Data to develop demographic information regarding its Site users. Such aggregated Usage Data and Personal Data will not identify you personally. COH may share this aggregated information with our partners, sponsors, affiliates, and other third-parties.
COH may publish lists of our donors in our annual report or online. These lists may include general categories of overall giving. Donors who wish to give anonymously should email development@cohdc.org.
If you provide your postal address, telephone number, or email address to the COH online, you may receive any of the following periodic contacts from COH:
• Mailing or telephone contact to thank you or invite you to support or volunteer for COH, to help COH achieve its mission, or to provide information about COH’s programs and events.
• Email contact to respond to your questions, comments, or suggestions; alert you to COH news updates and programs; invite you to COH events; invite you to help COH achieve its mission; or to support or volunteer for COH. For information about how to opt-out of these communications, see below.
Users may always “opt out” at any time if receiving messages is no longer desired.
Privacy of Client Information
Securing and protecting client and patient information is a top priority for Community of Hope. COH serves vulnerable client populations and works to protect the privacy of our clients and patients.
To that end, photographs and images found on this Site are not available for third-party use. Partner organizations and funders occasionally request to use images to facilitate communication of Community of Hope’s mission and said partnership. Community of Hope will make every effort to attain approval from clients and patients before the disclosure of images to third-parties. Any other use of the images by third-parties is not permitted. Community of Hope reserves the right to refuse offers from media featuring our staff and/or clients when the final publication will not be available in English.
Opt Out: Your Control Over Our Collection and Use of Your Information
You may set your browser to (1) inform you when cookies are being set before a cookie is stored; (2) not accept cookies at all, and; (3) erase all cookies from your hard drive. However, if you choose to reject or erase cookies, your ability to navigate our Site may be limited.
You may choose not to provide Community of Hope with any contact information. However, without such information, we will be unable to respond to your request for information from and about COH or to otherwise communicate with you.
If you would like to opt-out of receiving some or all further information from Community of Hope, either (1) take the opportunity to opt-out wherever Personal Data is collected, or (2) take the opportunity to opt-out in links on the bottom of an email we send to you.

Any material changes to the policy will be effective thirty (30) calendar days following such notice to prior users of the Site and will be effective immediately for new users of the Site. Continued use of our Site following notice of any such changes shall indicate your acknowledgment of such changes.
