At Community of Hope, heart health takes center stage, and two new pilot groups are bringing patients together for engaging and informative sessions.
Patients like Marcia Daniels, 65, and Shani Toor, 30, have both benefited from these groups, each finding unique value in their participation.
For Marcia, joining the Heart Health Centering program at our Family Health and Birth Center has been a vital part of her journey toward rebuilding strength and health after a stroke in 2019. Paired with a dedicated Community Health Worker (CHW), Marcia receives personalized support to overcome barriers to care. The Heart Health Centering sessions, developed and co-facilitated by our Community Health Worker team and Family Nurse Practitioner Melanie Vaughn, include interactive games and activities, make learning about heart health an enjoyable experience.

Ellie Spector, Community Health Worker, explains, “We’ve covered everything from hypertension basics to stress management and nutrition tips. It’s a collaborative approach that encourages patients to share and learn together.”
Additionally, at the Marie Reed location, a new Healthy Living Group has emerged. Shani Toor, a member, emphasizes the importance of accountability in the group. “Having a class once a week forces you to think about your nutrition,” says Shani, highlighting how the weekly commitment reinforces the importance of prioritizing health daily.
Dr. Asha Subramanian, who spearheads the Healthy Living Group, stresses the significance of prevention. “We can prevent many chronic illnesses through education and proactive measures,” she affirms, emphasizing the need for greater awareness of preventable health issues.
These initiatives, including the Heart Health Centering program and the Healthy Living Group, highlight Community of Hope’s comprehensive approach to healthcare. From tailored support provided by dedicated CHWs to empowering educational sessions, Community of Hope remains committed to supporting patients like Marcia and Shani to continue on their journeys toward healthier hearts and happier lives.
