We are always adding plans, so please call us to see whether we accept your plan or whether we can arrange an out-of-network agreement (for birthing services). We also welcome uninsured patients, who may be eligible for a sliding fee scale payment plan.

No one will be denied access to services due to inability to pay, lack of insurance, or high out-of-pocket costs. There is a discounted/sliding fee schedule available based on family size and income. The sliding fee scale is available for all of our services. Eligible patients are assigned a level depending on their family income. Each level has different fees that are much lower than Community of Hope's normal fees. Just ask a Receptionist to help you get in touch with an Enrollment Assister to apply. We will tell you what you can use for proof of income to qualify for a discounted rate.
