COMMUNITY OF HOPE OUTCOMES

Health:

  • Community of Hope serves over 4,000 patients a year across our medical, dental and mental health components. In 2008, that represented almost 11,000 medical visits, 2,500 dental visits, and 300 mental health visits.
  • 93% of patients have incomes at or below 200% of the poverty level.
  • Health outcomes
    • 97% of two year olds had all appropriate immunizations.
    • 80% of sampled diabetic patients had controlled blood sugar levels
    • 64% of sampled patients with hypertension had controlled blood pressure

Housing:

  • In 2008, we provided housing and supportive services to 54 families in our two transitional housing programs at Girard Street and Hope Apartments. This represents 75 parents and 170 children.
  • 89% of those families moved successfully into affordable transitional or permanent housing.
  • In 2009, we are providing permanent housing and supportive services to an additional 52 families. 100% of families have maintained their housing to date.

Education

  • In 2008, we served 205 children in our housing programs, providing them and their parents with advocacy in the schools, connections to after-school programs and tutors, and fun, safe activities on the weekends.
  • At the end of the school year in June 2009, 94% of these children advanced to the next grade in school. Homeless children traditionally are behind in school and have problems advancing.
  • 51% of adults in our housing programs worked or were involved in job training during their stay.
