MEDICAL CASE MANAGER
Employer: Community of Hope is a nonprofit Federally Qualified Health Center serving the uninsured and underinsured populations in the District of Columbia.
Job Description: The Medical Case Manager position is full time (40 hrs/week). The job is divided between two programs: 1) HIV/Ryan White and 2) Social Services. HIV case management responsibilities include referrals and follow-up to appropriate medical and social service resources. May also require accompanying clients to some appointments. Social Service case management covers comprehensive service referrals, including housing, education, assisting clients with benefit applications, and arranging home-healthcare and other services as needed. Job duties would also include administering the Patient Prescription Plan, working in conjunction with the Head Nurse. COH serves homeless and low-income DC residents, including a large immigrant community. Nice working environment, great team, interesting work.
Education: B.A./B.S. in Social Work or related field or equivalent experience required.
Qualifications: Minimum two years work experience in health case management or social work. Prefer social work background, licensed in DC, but will consider comparable experience. Knowledge and background working with people with HIV/AIDS required.
Additional Requirements: Certified Addition Counselor certification required. Bilingual candidates (English, Spanish, and/or Amharic) strongly preferred.
