Youth Specialist (Full-Time)


The Youth Specialist is accountable directly to the Team Coordinator who oversees his/her team. This full-time position is responsible for working with the Housing Stability Case Managers, schools, and other community resources to ensure that the children on his/her caseload receive the educational and support services needed.

Team Coordinator - Rapid Re-Housing (Full-Time)


The Rapid Rehousing Team Coordinator is accountable directly to the Rapid Re-Housing Program Director. In conjunction with the Program Director, this position is responsible for providing supervision, administrative program support and oversight of a team of 4 housing stability case managers. In addition, the Team Coordinator will carry a caseload of up to 12 families providing individualized case management services to families as they work to maintain their housing and increase their income. Specific duties may include, but are not limited to, conducting program intakes and assessments, creating case management plans, data entry and chart reviews, facilitating recertification of housing subsidies, assisting with housing search and relocation process as needed and making appropriate referrals to community resources.

Team Coordinator - Home Now (Full-Time)


The Team Coordinator is accountable directly to the Home Now Program Director. In conjunction with the Program Director, this position is responsible for providing supervision, administration and oversight to a team of Housing Stability case managers and/or Youth Specialists. In addition, the Team Coordinator will carry a small caseload (up to 5 families) and provide both case management and some youth advocacy services. These duties will include performing intakes, creating case management plans that engage families in pursing strengths-based goals, assisting with housing search and relocation process, making appropriate referrals to support family stability, and working with schools and other community resources to ensure that the children on his/her case load receive the educational and support services needed.

Assessment Specialist (Full-Time)


The Assessment Specialist is responsible for screening applicants for eligibility and prioritization for rapid re-housing and permanent supportive housing (PSH). Using an assessment tool to evaluate appropriateness for rapid re-housing services and PSH services, the Assessment Specialist will make appropriate referrals and work closely with the Community of Hope team, as well as staff from other providers, to help families in shelter access and retain permanent housing (rapid re-housing, PSH, or other affordable housing). The Assessment Specialist may also assess families presenting as homeless or precariously housed at one or more of the District’s family resource centers.

Housing Stability Case Manager


The Housing Stability Case Manager will provide housing stability and income-focused case management services to the families assigned to their case load including; providing resources to increase income; meeting regularly with client, advocating for client housing needs, collaborating with Employment Specialist and families on completing resumes and job searches; and making referrals for appropriate services. This position reports to the Program Director.

Resident Monitor

Resident Monitors are integral to the success of Community of Hope's Housing Program! Resident Monitors are currently needed for Afternoon, Evening and Overnight shifts in COH's residential shelter program. Relief positions have the potential of leading to a full time position with benefits. Applicants must possess a strong desire to work with homeless families. Some weekend and holiday work required. This position reports to the Program Manager. This is a relief/casual position.
