Family Case Manager (Homeless Prevention and Rapid Re-Housing Program)
Community of Hope is currently looking for a full time Case Manager for its Homelessness Prevention and Rapid Re-Housing Program (HPRP). We are a nonprofit providing healthcare, housing, and supportive services to the low-income and homeless populations in the District of Columbia. We are looking for someone who is innovative, compassionate, and has a strong desire to work for the success of those in the housing program.
Job Description: The HPRP Case Manager is accountable directly to the HPRP Program Manager. This position is responsible for providing housing search and placement, housing stability, and income-focused case management services to the families assigned to his/her case load. Specific responsibilities include but are not limited to the following: helping families move into stable housing, providing 3-12 months of case management services to ensure that families are receiving necessary supportive services, helping connect families with resources to increase income, meeting weekly and as needed with families on case load to develop case plans, review progress, and to do follow up on tasks and services, collaborating with Housing Specialist and families on housing searches and placement processes, completing assessments, coordinating with community resources for housing, employment, substance abuse treatment, mental health services, and maintaining files and data for families on case load.
Education: B.A./B.S. in Social Work or related field OR equivalent work experience. MSW preferred. BSW/MSW degree holders must obtain a LSWA/LGSW or appropriate social work license within 90 days of hire.
Additional Qualifications: Minimum one year social work experience working with families, homelessness, and/or substance abuse strongly preferred. Strong interpersonal, organizational and conflict resolution skills required. Knowledge of community resources or the ability to become knowledgeable required.
