Family Case Manager
Community of Hope is currently looking for a full time Family Case Manager for its Housing Program. We're a nonprofit providing healthcare, housing and supportive services to the low-income and homeless populations in the District of Columbia. We are looking for someone who is innovative, compassionate and has a strong desire to work for the success of those in the housing program.
Job Description: The Family Case Manager is accountable directly to the Program Manager. This position is responsible for providing housing search and placement, housing stability and income-focused case management services to the families assigned to his/her case load. Additional responsibilities include but are not limited to the following: help families move into stable housing, help families with resources to increase income, meet weekly and as needed with families on case load to develop case plans, review progress and to do follow up on tasks and services, collaborate with Housing Specialist and families on housing searches and placement process, complete assessments, coordinate with community resources for housing, employment substance abuse treatment, mental health services and training, make referrals for appropriate services, meet regularly with Program Manager for supervision and communication of family needs and maintains files and data for those on their case load.
Education: Bachelor’s degree in social work or related field required. BSW/ MSW degree holders must obtain a LSWA/LGSW or appropriate social work license within 90 days of hire.
Additional Qualifications: Minimum three years work experience in social work or related field. Experience working with families, homelessness, and/or substance abuse strongly preferred. Knowledge of community resources or the ability to become knowledgeable required. Strong interpersonal and organizational skills required.
